We are a bit confused about the proper place to enter information about diabetic patients. The problem is caused by a recent requirement of Medicare that both the name of the primary care physician and when last seen by the primary care physician must be entered when filing a claim. It is not clear where on the form this information must be entered. Does it go under \"Case -- Miscellaneous -- Primary Care Provider Outside this Office?\"
